My five year old nephew

I have a nephew who turned five in may of this year. He isn't talking in complete sentences as he should be. He also chews on shoes and other various objects. He is on adhd medicine but it isn't working. I think there is something else wrong but no one else will acknowledge it. He screams at random times and sometimes will look like he is just lost. If anyone could help that would be great.
