Help... my daughter is getting worse

She was diagnosed at 4 and she is 5 now.
some days we think it's good then others we fall back again. she started school, the CBTU school for kids like her and she has been so stressed, that she is getting violent and know shreds her clothes. we have tried medications and putting her on a diet and keeps everything the same daily, it's just getting worse.
